A Spatial study of Keshan disease prevalence and Selenoprotein P in Heilongjiang Province

Background: The spatial epidemiology of Keshan disease (KD) prevalence with selenoprotein P (SEPP) is not well documented. Therefore, this study aimed to investigate the relationship between KD prevalence with serum SEPP levels and socio-economic status for evaluation and targeting of KD prevention, control, and elimination efforts.
Methods: Using a case-finding and key investigation study design, all 66 counties in Heilongjiang province where KD is endemic were surveyed. The serum SEPP levels from subjects were measured using enzyme-linked immunosorbent assay. Data regarding KD prevalence and influencing factors were collected using questionnaires. Thematic maps were created, and spatial regression analysis was performed by the ordinary least-square method.
Results: During 2013-2014, a total of 53,676 residents were investigated among the people in 66 KD endemic counties in Heilongjiang province. In total, 50 chronic KD cases were detected from case findings with a total prevalence rate of 9.3/10,000 (50/53,676). The prevalence rate in Tangyuan county was the highest (250/10,000 population). No new cases were found. Spatial regression analysis showed that the KD prevalence rate was positively correlated with SEPP levels (t=13.08, P<0.001), and negatively correlated with per capita disposable income of rural residents (t=-7.58, P<0.001).
Conclusion: Tangyuan county should be considered a key area for targeted KD prevention, control, and elimination. It is necessary to expand the sample size to further study the molecular markers of KD.
