Analysis of unsafe acts in the development of derailing accidents of inclined shaft trip lifting in coal mine

In order to reduce unsafe acts in derailing accidents of inclined shaft trip lifting in coal mine, by applying the accident causation “2-4”, the unsafe acts of 55 derailing accidents were studied by statistical analysis and classification. The results showed that derailing accidents resulting in casualties required to satisfy three conditions: vehicles from the mechanical control; failure of the protective facilities; persons in the danger zone. The most critical unsafe acts causing the vehicle to spin out of control including: not stopped lifting work when the rope was found badly worn; Use of non-locking device connecting pin; not the tub plug is inserted in the end; failing to deal with risks of broken wires in wire ropes overrun. The main unsafe acts causing the failure of the protective facilities including: untreated car guards potential failure; upgrade hang without safety lines; not installed protective devices. Accidents caused by casualties’ unsafe acts accounted for 74.55% of the total number of accidents. It includes two types of unsafe acts: take vehicles illegally; wait, walk or work in hazardous locations.
